What is a Senior Care Consultant?
A knowledgeable specialist who assesses, coordinates, and manages a plan for the continued
care of a Senior person depending upon need and request.
Who hires a Senior Care Consultant?
Adult children with aging parents are a primary referral source for Senior Care Consultants. A large number of aging parents no longer live near their children. When a medical crisis occurs or when help is needed to coordinate assistance, family members can rely on a consultant. Recognizing the benefits of utilizing a consultant for care management can positively impact health and well being, and insurance companies will understand these benefits. Long term care insurance companies need assistance when home care assistance or other long term care options become necessary. The Senior Care Consultant can offer a home visit evaluation to determine options for the most appropriate level of care.
What are some of the services a Senior Care Consultant might provide?
- In-person assessment and development of client-specific care management plan
- Collaboration with all parties: client, family, physicians, ancillary providers, medical equipment providers, etc.
- Coordination of and attendance at medical appointments
- Coordination of needs prior to hospital or facility discharge
- Assistance in locating and selecting in-home providers and/or long term facility care
- Assistance in locating and selecting ancillary providers for such needs as medical equipment, housekeepers, etc.
