- Assistance in locating and selecting in-home providers and/or long term facility care
- Assistance in locating and selecting ancillary providers for such needs as medical equipment, housekeepers, etc.
- Identification and coordination of free services, when available and applicable
- Pet care coordination
- Education of client, spouse and family as appropriate regarding all aspects of care coordination
- Assessment of medications and discussion with client, spouse and family as appropriate and prescribing physician if issues or questions arise
